SJC oversees multiple facets of the Undergraduate Student Government disciplinary process. Use the forms below to report misconduct or provide anonymous feedback. To learn more about our hearings process regarding complaints and appeals and to submit a request for an advisory opinion, click here.

USG Reporting Form

If you are aware of any misconduct by a USG member, please use this form to report it to the Chief Justice and the Advisors for investigation and review.

Appeal a Parking Ticket

If you feel that your parking citation was issued contrary to the Parking and Transportation Regulations or was simply issued in error, you may appeal your parking citation by submitting this form. For step-by-step instructions for filing appeals, see the Parking Portal Information Page. Appeals must be submitted within ten (10) calendar days of the issuance of the citation.
